Community Blue PPO

Community Blue PPO, administered by Blue Cross Blue Shield of Michigan, offers the largest provider network in the U.S. and allows you to see any provider or specialist without a referral or choosing a primary care physician. This flexibility, including access to out-of-network care and global coverage, comes with higher monthly premiums compared to other U-M health plans.

You’ll save on out-of-pocket costs by using in-network providers, but you retain the option to seek care anywhere, making this plan particularly suitable for those who travel or live outside Michigan. As with all U-M health plans, prescription drug coverage is included with enrollment.

Important Plan Details

Offers the flexibility to see any provider or hospital throughout the U.S. without a primary care referral.
You are not required to select a Primary Care Provider (PCP); however, a PCP acts as a "medical home," providing preventive care and screenings, managing chronic conditions, and coordinating care across different specialists.
You have the option to use out-of-network providers, but you will receive the greatest savings by choosing BCBSM PPO national providers.
Out-of-network services are generally covered at 50% of BCBSM fee schedule. Out-of-network providers may charge more than the fee schedule allows. You are responsible for 100% of charges in excess of BCBSM reimbursement.
In-network preventive services are fully covered; preventive services from out-of-network providers are not covered.
You are covered at the in-network benefit level for approved services while traveling outside the U.S. using BCBS GlobalCore providers.
Ideal for individuals who live or travel outside Michigan, or for those seeking coverage both within the U.S. and around the globe.
