Medicare Advantage PPO

Medicare Advantage PPO, also referred to as Medicare Plus Blue Group PPO, is administered by Blue Cross Blue Shield of Michigan. PPO stands for Preferred Provider Organization with a nationwide network of quality providers.

Medicare Advantage PPO provides flexibility in choosing health-care providers without needing referrals. As with all U-M health plans, prescription drug coverage is included with enrollment.

Prescription drug coverage provided by Prime Therapeutics is included with the plan.

No annual deductible
No out-of-pocket cost to you for preventive care
$10 copay for office visits for injury or illness
$10 copay for specialist visit
$10 copay for urgent care
$65 copay for emergency care (waived if admitted)
$10 copay for mental health, behavioral health and substance abuse services
No out-of-pocket cost to you for inpatient services
No out-of-pocket cost to you for outpatient surgery
$3,000 individual/$6,000 family out-of-pocket maximum

Important Plan Details

Must live in the plan’s service area, which is within the U.S.
In-network providers are located throughout the U.S.
Out-of-network, medically necessary services are covered when provided and billed to the plan by a Medicare-participating provider.
You do not need a referral to see a specialist.
The plan must cover all services covered by original/traditional Medicare and follow original/traditional Medicare’s coverage rules.
Covers emergency services when traveling internationally.
