Workforce applicants of Michigan Medicine and select campus departments following Michigan Medicine Policy 04-06-002 will be required to complete the Pre-Placement Health Assessment as a condition of employment. Applicants should expect to receive a Welcome to the University of Michigan Onboarding Portal email that will contain a unique portal activation code and instructions to create an Enterprise Health portal account.
Within the Enterprise Health applicant portal, applicants will click on the red bell to see the activities that need to be completed. Within the portal, the applicant will complete the assigned questionnaires to submit historical immunization records and will schedule an appointment to visit Occupational Health Services (OHS) to fulfill any outstanding job requirements. Pre-Placement Health Assessment appointments are available at our convenient offsite location: 2705 South Industrial Highway, Suite 400, Ann Arbor, Michigan.
Please note: You may be required to provide a blood sample to assess immunity status during your pre-placement health assessment. Please arrive well hydrated and eat a light meal 1-2 hours before your appointment.
Applicants should login to the Enterprise Health applicant portal, complete the questionnaire and schedule the Pre-Placement Health Assessment appointment as soon as possible to fulfill all the requirements necessary to proceed in the Human Resources hiring and on-boarding process.
