Employee:

1. Locate the Reinstatement or Credit for Prior Years of Service Form. Note: Ann Arbor Faculty and Staff may submit the request electronically.
   • Complete the form.
   • Submit it for review.

Appropriate HR Department (Michigan Medicine Solution Center, Dearborn Human Resources, Flint Human Resources, Ann Arbor SSC):

1. Complete review and notify employee.
