Procedure A: Requesting sick leave income and reporting time
Faculty Member:
If the period of incapacity has exceeded or will exceed three calendar weeks, submit written request for sick leave with appropriate supporting documentation (e.g., physician's letter) to department chair, academic program director, or dean.
Department chair, academic program director, or dean (or his or her designate):
- Review request to determine whether the faculty member is eligible for sick leave.
- If the request is approved, modify the appointment accordingly and notify the faculty member and Human Resources Records and Information Services (HRRIS).
Procedure B: Returning to work (when the faculty member did not know his or her return date when he or she requested sick leave income)
Faculty Member:
- At an appropriate time prior to the faculty member's return to work, notify department chair, academic program director, or dean of the date on which the faculty member intends to return to work.
- Provide a medical statement from physician approving a return to work.
Department chair, academic program director, or dean (or his or her designate):
Modify the appointment accordingly and notify the faculty member and Human Resources Records and Information Services (HRRIS).
