GENERAL QUESTIONS

What is MHealthy Rewards?

MHealthy Rewards is a free, voluntary and confidential program that helps qualifying benefits-eligible faculty and staff and their U-M health plan enrolled spouse/other qualified adult (OQA) learn about their health and then take steps to improve or maintain it. Offered annually, MHealthy Rewards includes completing a confidential health questionnaire to receive a personalized health report. To maintain confidentiality, the University of Michigan partners with StayWell® Health Management, a national provider of health improvement services to provide the health questionnaire.

How do I participate in MHealthy Rewards 2015?

For eligible employees, MHealthy Rewards 2015 includes two steps:

  1. Complete the 2015 StayWell confidential health questionnaire by March 13, 2015.
    AND
  2. Complete a Rewards-eligible health and well-being activity by June 26, 2015.

For eligible spouses/other qualified adults, MHealthy Rewards 2015 includes:

  • Completing the 2015 StayWell confidential health questionnaire by March 13, 2015.
  • Learning more about health and well-being programs you can join.

Why is the university offering MHealthy Rewards?

MHealthy Rewards helps eligible participants learn helpful information about their health and then take steps to improve or maintain it. The university is deeply committed to creating and encouraging a culture of health among its faculty and staff community. The goal of programs like MHealthy Rewards is to encourage sustainable healthy lifestyles that help to prevent chronic disease and other health conditions, and manage rising health care costs.

Why should I participate in MHealthy Rewards?

By understanding your current health and risk factors, you can take action to help minimize or prevent future health problems.

How much does it cost?

Taking the health questionnaire is free as well as most eligible health and well-being activities – the vast majority of participants will complete MHealthy Rewards without incurring any costs.

ELIGIBILITY

Who can participate in MHealthy Rewards?

Qualifying benefits-eligible faculty and staff and their enrolled spouse/other qualified adult (OQA) can participate in MHealthy Rewards. Find specific eligibility requirements here or call SSC Contact Center at (734) 615-2000.

Please note: If at any time the eligible employee's benefit status changes (leave of absence, appointment change, etc.), the employee and their enrolled spouse/OQA's eligibility to participate, and/or eligibility for any monetary reward, may also change.

Can retirees participate in MHealthy Rewards?

No, retirees are not eligible for MHealthy Rewards.

Can temporary employees participate in MHealthy Rewards?

No, temporary employees are not eligible for MHealthy Rewards.

I have never participated in the MHealthy Rewards, can I participate in this year's program?

Yes, if you are a qualifying benefits-eligible faculty or staff, you can participate in MHealthy Rewards even if you did not participate in the previous years.

Can my dependents participate in MHealthy Rewards?

No, dependents are not eligible for MHealthy Rewards.

HEALTH QUESTIONNAIRE

How do I access the online 2015 health questionnaire?

Simply go to www.MHealthy.umich.edu/rewards.

Benefits-eligible faculty and staff: Select the benefits-eligible faculty or staff option and log in using your U-M unique name and Kerberos/Level 1 password. Your first time into the StayWell website in 2015, you will need to complete a brief, one-time registration process.

Spouses and other qualified adults enrolled in a U-M health plan: Select the enrolled spouse/other qualified adult option, and then follow the prompts. Please note, you will need your spouse's/OQA's eight-digit U-M employee ID number.

For those without computer access, you can request a paper health questionnaire by calling StayWell at 1-800-543-6717.

How long will it take to complete the health questionnaire?

The 2015 health questionnaire takes approximately 10 minutes to complete.

Do I need to accept a disclaimer or release to complete the health questionnaire?

Yes, you will have to accept StayWell's Terms of Use and Privacy & Security Statement.

How do I complete the health questionnaire?

Answer the questions and click "Finish & View Results" at the end. If you do not click "Finish & View Results," your questionnaire is not considered complete.

Do I have to answer all the questions on the health questionnaire for it to be considered complete?

You will have to answer most questions on the health questionnaire, although some are not required.

What if I don't complete the health questionnaire?

To participate in MHealthy Rewards, you will need to complete the health questionnaire.

I completed a health questionnaire for MHealthy Rewards previously (2009, 2010, 2011, 2012, 2013 and/or 2014). Can I do another one?

Yes, we encourage all qualifying benefits-eligible faculty and staff and their spouses and other qualified adults enrolled in a U-M health plan to complete the health questionnaire annually to see how their health changes from year to year.

Do I have to complete the health questionnaire before I begin a Rewards-eligible health and well-being activity?

No, you do not have to complete the health questionnaire before you begin an eligible activity. However, you will not receive the $50 before-tax incentive for completing your eligible activity until your 2015 health questionnaire is completed.

What if I need assistance in completing the health questionnaire?

Please call StayWell at 1-800-543-6717, Monday - Thursday 9 a.m. to 9 p.m., Friday, 9 a.m. to 7 p.m., Saturday 9 a.m. to 1 p.m. for assistance with completing the health questionnaire.

BENEFITS-ELIGIBLE EMPLOYEES – ELIGIBLE HEALTH AND WELL-BEING ACTIVITIES

What are the eligible health and well-being activities for MHealthy Rewards 2015?

To view the entire list of Rewards-eligible activities for faculty and staff, go to www.MHealthy.umich.edu/rewards and click on the option for benefits-eligible faculty and staff, then log into the StayWell website and click on the “My Rewards” tab.

Do I need to have my cholesterol and blood pressure measurements for the MHealthy Rewards 2015 program?

No, this year, MHealthy Rewards does not include biometric values like blood pressure or cholesterol. You will not need to attend an on-site screening or get these values from your physician to complete the program. To participate in MHealthy Rewards 2015, eligible employees can:

  • Complete the 2015 StayWell confidential health questionnaire by March 13, 2015 to earn a $50 before-tax reward.
    AND
  • Complete a Rewards-eligible health and well-being activity by June 26, 2015 to earn another $50 before-tax reward.

How does MHealthy know I am participating in an eligible activity?

If you are enrolled in a Rewards-eligible activity, please be assured that each activity has processes in place to track your participation. If it has been longer than 60 days since you have completed your eligible activity and your health questionnaire, please email mhealthy@umich.edu for additional information.

How does my activity count towards MHealthy Rewards?

In order for a Rewards-eligible activity to count, you must meet the completion criteria for the activity in which you participate. Each activity has its own completion criteria; please make sure to look for information when you begin your program.

What if I don't complete my health and well-being activity?

If you do not complete one Rewards-eligible health and well-being activity by June 26, 2015, you will not receive the additional $50 before-tax Reward. Remember to plan ahead, as you will need to begin most activities by April 2015.

Is Active U part of MHealthy Rewards?

Yes, Active U is a Rewards-eligible activity. To participate in Active U, you must sign up by 5 p.m., February 9.

I am participating in a health and well-being activity that is similar to one of the Rewards-eligible activities. Does this qualify?

No. For data collection and data integrity purposes, only specific programs qualify for MHealthy Rewards 2015.

Can I continue to participate in MHealthy health and well-being activities after I've completed MHealthy Rewards?

Absolutely! But please note that you cannot earn more than $100 before-tax reward ($50 for health questionnaire and $50 for an eligible activity).

I want to participate in a specific health and well-being activity, however it is currently full.

Space and registration is limited for some activities. We encourage you to look for another eligible activity if the one you are interested in is currently unavailable. For a complete list of Rewards-eligible health and well-being activities, log into the StayWell site and click on the “My Rewards” tab.

CONFIDENTIALITY

Who is administering the health questionnaire for MHealthy Rewards?

Information provided within the MHealthy Rewards program is treated as confidential. To protect your privacy, the University of Michigan has contracted with StayWell® Health Management, a national provider of health improvement services, to administer the health questionnaire, health coaching and online programs.

StayWell specializes in helping organizations improve the health and well-being of their employees.

What if I don't agree with the StayWell's Terms of Use or Privacy & Security statement?

You will need to agree to both the StayWell’s Terms of Use and Privacy & Security statement in order to participate in MHealthy Rewards. If you do not agree, you will not be allowed to complete the health questionnaire, which is required to participate in the program.

Will I be penalized if I don't participate?

No, participation is voluntary.

Will the results of my health questionnaire affect my insurance coverage or benefits?

No, your personal health information is protected. Your results will not affect insurance coverage or benefits nor will they be disclosed to your insurance company.

Will any of the information obtained through my health questionnaire be shared with my manager?

No, the confidentiality of your personal health information is protected. Your personally identifiable health data will not be shared with your manager, nor will it be added to any U-M faculty or staff files.

MONETARY REWARD (for benefits-eligible U-M employees only)

Is there a monetary reward for participating in MHealthy Rewards?

Yes, qualifying benefits-eligible faculty and staff can earn up to $100 (before taxes) in their paycheck:

  • Complete the 2015 StayWell health questionnaire to earn a $50 before-tax Reward.
  • Then complete ONE eligible health and well-being activity by June 26, 2015 to earn an additional $50 before-tax Reward.

There is no monetary incentive for eligible spouses and other qualified adults.

Why is U-M giving employees $100 for participation in MHealthy Rewards?

Research has shown that financial incentives and a supportive workplace are effective ways to achieve high levels of participation. Research has also shown that high participation in wellness programs achieved through the use of incentives also generates cost savings for the organization and its employees. Since the university has been seeing steady increases in health care costs for the last decade, U-M has made a long-term commitment to the health and well-being of its faculty and staff to first improve their health, but also to save on health care costs.

How will I receive my reward?

Rewards payments will be paid before taxes and will occur within your monthly or biweekly U-M paycheck.

Where can I see my MHealthy Rewards payment?

To see your MHealthy Rewards payment, please log into Wolverine Access to view your paycheck. Click on your most current paycheck and look for an additional line item entitled "MHealthy Program Credit" under the "Hours and Earnings" section. This is where your payment can be seen.

If the payment shows up in the YTD (year to date) column, then your payment for MHealthy Rewards has processed on a previous paycheck. If it is in the Current column, then your payment has occurred on the paycheck you are viewing.

For additional assistance, please talk with your department payroll administrator or call the SSC Contact Center at 734-615-2000 for additional assistance.

When will I receive my monetary reward?

Health questionnaire payments can take up to 60 days to be paid. For activity payments, it may take up to 30-90 days after completion of an activity.

What if I don't want to receive the monetary reward?

If you do not want to receive the reward, we suggest that you donate the incentive to a charity of your choice. If you would like your donation to benefit the university, you may want to consider Project Healthy Schools, a school-based program sponsored by U-M to help reduce childhood obesity and its long-term health risks, or the U-M Hardship Program, which helps U-M employees who are having financial difficulties, or one of the Health System's programs.

Where can I go if I don't have easy access to a computer?

A computer is not required to participate in MHealthy Rewards. To request a paper health questionnaire, please call StayWell at 800-543-6717.
