GENERAL QUESTIONS
- How do I sign up for MHealthy Rewards?
To participate in MHealthy Rewards, go to www.mhealthy.umich.edu/rewards on or after Tuesday, January 24, 2012, and log in using your U-M unique name and Kerberos password. Once logged in, you will be able to complete your health questionnaire and schedule an on-site screening appointment.
For those without computer access, you can request a paper health questionnaire and schedule an on-site screening appointment by calling StayWell at 1-800-543-6717 and selecting option 1 for the HelpLine.
- What is MHealthy Rewards?
MHealthy Rewards is a free, voluntary and confidential program that helps qualifying benefits-eligible faculty and staff learn helpful information about their health and then take steps to improve or maintain it. In 2012, MHealthy Rewards includes completing the annual health questionnaire and completing a wellness screening to know your numbers for cholesterol, blood pressure and body mass index. The screening concludes with a brief results review session where you can review your personal health report with a health professional and create an action plan. Both the health questionnaire and on-site wellness screenings are administered by StayWell® Health Management, a national provider of health improvement services.
- When does MHealthy Rewards begin and end?

MHealthy Rewards starts January 24, 2012. You must complete the 2012 annual health questionnaire and wellness screening by May 2012 to receive the $100 reward.
- How do I complete MHealthy Rewards?
To complete MHealthy Rewards 2012 you must do the following by May 2012:
- Complete the confidential 2012 StayWell health questionnaire
AND - Complete a confidential wellness screening (options are available)

- What options are available to complete MHealthy Rewards in 2012?
- To complete MHealthy Rewards in 2012, qualifying benefits-eligible faculty and staff must complete the health questionnaire and complete a wellness screening. Online and paper options are available to complete the health questionnaire. Options to complete the wellness screening include attending an on-site screening or using values (height, weight, blood pressure, total and HDL cholesterol values) from your health care provider (you will need to submit a health care provider form). - Why is the university offering MHealthy Rewards?
- The university is deeply committed to creating and encouraging a culture of health among its faculty and staff community. The goal of programs like MHealthy Rewards is to encourage sustainable healthy lifestyles that help to prevent chronic disease and other health conditions, and manage rising health care costs.
- Why should I participate in MHealthy Rewards?
- By understanding your current health, knowing your numbers, and risk factors, you can take action to help minimize or prevent future health problems.
- How long will it take to complete MHealthy Rewards?
- The 2012 annual health questionnaire takes less than 20 minutes to complete and the on-site wellness screening takes approximately 40 minutes to complete.
- How much does it cost?
- Taking the health questionnaire and attending an on-site wellness screening are free.
- Am I required to participate in MHealthy Rewards?
- No, participation is voluntary.
- What if I have questions?

- Can I still participate in the MHealthy Rewards if I am pregnant?
- Yes, you can participate in MHealthy Rewards even if you are pregnant. You will be asked about pregnancy on the screening consent form that you will fill out prior to your wellness screening appointment.
ELIGIBILITY
- Who can participate?
Qualifying benefits-eligible university faculty and staff can participate in MHealthy Rewards. Find specific eligibility requirements here or call HR/Payroll Service Center at (734) 615-2000 (please select option 1).
Please note: If at any time your benefit status changes (leave of absence, appointment change, etc.), your eligibility to participate and receive the monetary reward may change.
- I am a benefits-eligible employee but I do not get my benefits through U-M. Can I still participate in MHealthy Rewards?
- If you are a qualifying benefits-eligible employee that meets the MHealthy Rewards eligibility requirements, you can participate in MHealthy Rewards. Please note: If at any time your benefit status changes (leave of absence, appointment change, etc.), your eligibility to participate and receive the monetary reward may change.
- Can retirees participate in MHealthy Rewards?
- At this time, retirees are not eligible to participate in this program.
- Can part-time employees participate in MHealthy Rewards?
- Part-time employees that meet the eligibility requirements can participate in MHealthy Rewards. Please note: If at any time your benefit status changes (leave of absence, appointment change, etc.), your eligibility to participate and receive the monetary reward may change.
- Can temporary employees participate in MHealthy Rewards?
- No, temporary employees are not eligible for MHealthy Rewards.
- I have never participated in the MHealthy Rewards, can I participate in this year’s program?
- Yes, if you are a qualifying benefits-eligible employee, you can participate in MHealthy Rewards even if you did not participate in the previous years.
- Can my spouse/OQA/dependent participate in MHealthy Rewards?
- Currently, MHealthy Rewards is only available to qualifying benefits-eligible faculty and staff.
HEALTH QUESTIONNAIRE
- How do I access the online 2012 annual health questionnaire?
- Simply go to www.MHealthy.umich.edu/rewards on or after Tuesday, January 24, 2012 and log in using your U-M unique name and Kerberos/Level 1 password. You will be taken to the MHealthy Rewards secure home page. Click on "Complete Your StayWell Health Questionnaire." Once on the StayWell website, you may need to complete a brief, one-time registration process before taking the health questionnaire.
- How long will it take to complete the health questionnaire?
- The 2012 annual health questionnaire takes less than 20 minutes to complete.
- Do I need to accept a disclaimer or release to complete the health questionnaire?
- Yes, you will be asked to read and accept StayWell's privacy statement before taking the health questionnaire on the StayWell Online website.
- How do I complete the health questionnaire?
- Answer as many health questionnaire questions as you can and click "Finish & View Results" at the end. If you do not click "Finish & View Results," your questionnaire is not considered complete.
- Do I have to answer all the questions on the health questionnaire for it to be considered complete?
- Though you do not have to answer all of the questions on the health questionnaire, to get the most accurate personal health report, it is recommended that you answer as many as you can. The only required questions on the health questionnaire are your height and weight.
- What if I don't complete the health questionnaire?
- To participate in MHealthy Rewards 2012 and be eligible to receive the monetary incentive, you must complete the 2012 health questionnaire (by clicking "Finish & View Results") and complete a wellness screening by May 2012.
- I completed a health questionnaire for MHealthy Rewards previously (2009, 2010, and/or 2011). Can I do another one?
- Yes, we encourage all qualifying benefits-eligible faculty and staff to complete the health questionnaire annually to see how their health changes from year to year. In order to participate in MHealthy Rewards 2012, you must complete the 2012 annual health questionnaire available beginning January 24, 2012. If you completed the health questionnaire in 2009, 2010 and/or 2011 you will receive a year-to-year personal health report comparison.
- Do I have to complete the health questionnaire before I complete my wellness screening?
- While it is not required to complete the health questionnaire before your wellness screening, it is recommended that you complete it prior to your on-site wellness screening appointment. Participants who have completed the online health questionnaire prior to their appointment will be able to review their questionnaire results with a StayWell health professional at the end of their on-site wellness screening appointment. Please note you will not qualify to receive the $100 before-tax reward until your 2012 health questionnaire and wellness screening are completed. Both the 2012 health questionnaire and wellness screening must be completed by May 2012.
- What if I need assistance in completing the health questionnaire?
- You can request a paper health questionnaire by calling StayWell at 1-800-543-6717 and select option 1 for the HelpLine, Monday - Thursday 9 a.m. to 9 p.m., Friday, 9 a.m. to 7 p.m., Saturday 9 a.m. to 1 p.m. StayWell helpline representatives can mail a paper health questionnaire to your home. For employees who may have a language barrier to completing the health questionnaire, please contact MHealthy at 647-7888 Monday – Friday from 8 a.m. to 4:30 p.m. to about options available.
WELLNESS SCREENING
- What is a wellness screening?
A wellness screening is a confidential assessment that gives you the opportunity to learn about your personal health. It includes having your height, weight, blood pressure, total cholesterol and HDL cholesterol values checked and then reviewing your results. A wellness screening can be completed by attending an on-site screening or using values (height, weight, blood pressure, total cholesterol and HDL cholesterol) from your health care provider.
Both wellness screenings options are being administered by StayWell to maintain the confidentiality of your personal health information.
Please note the on-site wellness screenings are not meant to be a substitute for your normal physician visit, but rather are intended to raise awareness about your health. StayWell’s health professionals will refer you to your physician if they feel you may need to follow-up on any results provided.

- How should I prepare for my on-site screening appointment?
- Bring your U-M Employee ID card (yellow card). No fasting is required. You can eat and take medications on your normal schedule. You can dress as you normally would for work. Please arrive on time and check in at the registration desk. Please note: if you arrive late for your appointment, we may not be able to accommodate your appointment time.
- Can I schedule my wellness screening during my work day?
- A variety of appointment times are available to accommodate work schedules during and outside of normal business hours. Because departments/units vary in their policies and staffing needs, please check with your manager or supervisor prior to making your appointment if it will occur during your scheduled work day.
- Are there on-site screening appointments available in the evenings and on weekends?
- Yes, evening appointments (those beginning 5:00 p.m. or later) will be marked as 'Screening - Evening Appointments' on the registration site. For Saturday appointments, the day of the week is listed under the 'Date' column, so you should search for those by the day listed.
- What if I miss my appointment or need to change it?
- The best way to cancel or reschedule your on-site wellness screening appointment is to open the confirmation email you received immediately after you scheduled your appointment. At the bottom of this email, there will be a sentence that says "Click here to cancel or reschedule your current appointment." This is a direct link to the registration system and will be the easiest way to reschedule your appointment. If you are unable to open or find this email, please call StayWell at 1-800-543-6717 Monday - Thursday 9 a.m. to 9 p.m., Friday, 9 a.m. to 7 p.m., Saturday 9 a.m. to 1 p.m. and select option 1 to reach the HelpLine.
- How will my cholesterol be checked at the on-site wellness screening?
- All tests are performed with just a simple fingerstick using the Cholestech LDX machine, which is the preferred system for cholesterol testing in the worksite setting. The accuracy, speed, simplicity and transportability of the Cholestech LDX allow for immediate testing and feedback on results, catering to the busy schedules of those in the workplace. Additionally, the Cholestech LDX matches the percent total error standards recommended by the National Cholesterol Education Program (NCEP), and is certified by the CDC's Cholesterol Reference Method Laboratory Network (CRMLN), the same requirements to which clinical laboratories are held.
- Do I have to fast before my on-site wellness screening appointment?
- No, you do not have to fast before your wellness screening, but it is recommended that you avoid eating or drinking (if possible) two hours prior to your appointment.
- Do I have to participate in all parts of the on-site wellness screening?
- No. You are encouraged to complete all parts of the screening, but if you are uncomfortable with any part of the screening, you may choose to opt out of that part of the process.
- I recently went to my health care provider and had my height, weight, blood pressure, and cholesterol checked, can I use those values?
- Maybe. If you had your height, weight, blood pressure, total cholesterol and HDL cholesterol tested between April 1, 2010 and April 1, 2012 and you print off the Health Care Provider form, have it completed and submit to StayWell by April 30, 2012, then you can use these to satisfy the requirement and complete a wellness screening. If you do not have all of these values, do not submit these values to StayWell using the Health Care Provider Form by April 30, 2012 or if your values were not collected between April 1, 2010 and April 1, 2012, then you are not able to use these values. - How can I print a copy of my Personal Health Report that was reviewed with me at the on-site wellness screening?
- To review and print a copy of your personal health questionnaire results from your wellness screening, go to www.MHealthy.umich.edu/rewards and log in using your U-M unique name and Kerberos/Level 1 password. You will be taken to the MHealthy Rewards secure home page. Click on "View Your Health Questionnaire Results".
- I left my paperwork at the on-site wellness screening. What should I do?
- If you left your pink participant consent form with your screening values on it, please call StayWell at 1-800-543-6717 and select option 1 for the HelpLine. If it was turned in, StayWell will mail it to your home.
CONFIDENTIALITY
- Who is administering the health questionnaire and wellness screenings for MHealthy Rewards?
- To protect your privacy, the University of Michigan has contracted with StayWell® Health Management, a national provider of health improvement services, to administer the health questionnaire, wellness screening, and health coaching. StayWell specializes in helping organizations improve the health and well-being of their employees. Since 1978, StayWell has designed and managed programs for employers throughout the United States.
- I used my U-M login ID and password to log into the MHealthy Rewards secure website, is my information still confidential?
- Yes. To simplify the Rewards login process and eliminate the need to create and remember another username ID and password, the university has worked with StayWell to make the health questionnaire and online Healthy Living Programs accessible using your U-M Login ID and Kerberos password. Your information continues to remain confidential.
- What happens with the information I provide?
- Information collected from MHealthy Rewards is treated as confidential. By agreeing to StayWell's privacy statement, you are allowing StayWell to view your health questionnaire information and advise you on health improvement strategies. The University of Michigan also utilizes a national leader in health care analytics and data management as its data warehouse vender. Data are stored using unique unidentifiable codes that are created by the vender. Those codes are not shared with the University of Michigan employees or external parties.
- What happens to the results and health information from the health questionnaire and wellness screening?
- Information collected from MHealthy Rewards is treated as confidential. StayWell will use the health information that you provide to advise you on health improvement strategies. StayWell treats your results as confidential. Completion information will be used to process your incentive. The University of Michigan also utilizes a national leader in health care analytics and data management as its data warehouse vender. Data are stored using unique unidentifiable codes that are created by the vender. Those codes are not shared with the University of Michigan employees or external parties. Only aggregate (group) de-identified data is used to help the university better understand the collective health of our faculty and staff and to assist with planning future programs that address the needs of our community. Please be assured that the confidentiality of your personal health information is protected.
- What if I don't agree with the StayWell privacy statement?
- You will need to agree to the privacy statement in order to participate in MHealthy Rewards. If you do not agree, you will not be allowed to complete the health questionnaire, which is required to participate in the program.
- Will I be penalized if I don't participate?
- No, participation is voluntary. However, the $100 before-tax reward is only available to those who complete the 2012 health questionnaire and wellness screening by May 2012.
- Will the results of my health questionnaire or wellness screening affect my insurance coverage or benefits?
- No, your personal health information is protected. Your results will not affect your insurance coverage or benefits nor will they be disclosed to your insurance company or your employer without your permission.
- Will any of the information obtained through my health questionnaire or wellness screening be shared with my manager?
- No, the confidentiality of your personal health information is protected. Your personally identifiable health data will not be shared with your manager, nor will it be added to any U-M faculty or staff files.
MONETARY REWARD
- Is there an incentive for participating?
- Yes, qualifying benefits-eligible faculty and staff who complete the 2012 annual health questionnaire and complete a wellness screening by May 2012 earn a $100 before-tax reward within their U-M paycheck.
- Why is U-M giving employees $100 for participation in MHealthy Rewards?
- Research has shown that financial incentives and a supportive workplace are effective ways to achieve high levels of participation. Research has also shown that high participation in wellness programs achieved through the use of incentives also generates cost savings for the organization and its employees. Given these tough economic times, and since the university has been seeing steady increases in health care costs for the last decade, U-M has made a long-term commitment to the health and well-being of its faculty and staff to first improve their health, but also to save on health care costs.
- How will I receive my reward?
- Rewards payments will be paid before taxes and will occur within your monthly or last biweekly U-M paycheck.
- Where can I see my MHealthy Rewards payment?
To see your MHealthy Rewards payment, please log into Wolverine Access to view your paycheck. Click on your most current paycheck and look for an additional line item entitled "MHealthy Program Credit" under the "Hours and Earnings" section. This is where your payment can be seen.
If the payment shows up in the YTD (year to date) column, then your payment for MHealthy Rewards has processed on a previous paycheck. If it is in the Current column, then your payment has occurred on the paycheck you are viewing.
For additional assistance, please talk with your department payroll administrator or call the HR/Payroll Service Center at 734-615-2000 and select Option 1. Since they can see your actual paycheck, they should be able to provide additional information.
- When will I receive my monetary reward?
- Your $100 before-tax reward payment may take up to 60 days from when you completed the MHealthy Rewards program (completing both the health questionnaire and wellness screening).
- What if I don't want to receive the monetary reward?
- If you do not want to receive the reward, we suggest that you donate the incentive to a charity of your choice. If you would like your donation to benefit the university, you may want to consider Project Healthy Schools, a school-based program sponsored by U-M to help reduce childhood obesity and its long-term health risks, or the U-M Hardship Program, which helps U-M employees who are having financial difficulties, or one of the Health System's research programs.
OTHER
- Is Active U a part of MHealthy Rewards?
- No, Active U is not part of MHealthy Rewards 2012. Though participation in Active U will not earn you a MHealthy Rewards incentive, being more physically active can help you achieve your health goals. Learn more about Active U at www.mhealthy.umich.edu/activeu
- Where can I go if I don't have easy access to a computer?
All three campuses offer computing sites for general use:

- What are the "MHealthy Programs" and "MHealthy Rewards" line items listed on my paycheck?
- The line items "MHealthy Programs" and "MHealthy Rewards" listed under the EMPLOYER PAID BENEFITS SECTION on your paycheck are amounts being contributed by the university (not deductions from your pay) each pay period to provide programs and incentives in areas such as physical activity, weight control and smoking cessation geared toward improving health and reducing health risks, as well as to provide faculty and staff assistance programs.
- I already see my health care provider? Why should I participate?
- This program is focused on prevention; it provides educational materials, referrals, and personal support with proven results. The materials and follow-up programs will help you reduce health risk factors and/or better manage certain chronic health conditions in partnership with your personal health care provider. The information is tailored to each individual's personal needs. Participants are encouraged to share their report with their health care provider.
