TITLE: NURSE PRACTITIONER -  HOME-BASED NURSING GRADE: HP
 

BASIC FUNCTION AND RESPONSIBILITY

To provide expert direct primary and specialty care to patients and families in the home; to provide health education to patients and family; to provide education for health care professionals.

CHARACTERISTIC DUTIES AND RESPONSIBILITIES

Assess, diagnose and treat health problems for home care patients.
Perform health and safety assessments, including patient history, physical and mental health examinations, diagnostic tests, medication management, symptom treatment, immunizations and other appropriate interventions or referrals.
Develop a plan of care in collaboration with the Medical Director and the patient's referring physician.
Initiate follow-up and/or transitional planning with the referring physician or agency.
Document patient care and progress and maintain complete and detailed patient records.
Provide health education to patients, families and other caregivers.
Present in-service programs.
Participate in quality improvement studies and program development to meet the needs of the community and facilitate patient care in the least restrictive setting.
Participate in agency committees and work teams.
Participate in agency sponsored educational and health programs and activities.

SUPERVISION RECEIVED

General supervision is received from the Medical Director or designee. 
Professional nursing practice is supervised by the Director of Patient Care Services or designee.

SUPERVISION EXERCISED

Functional supervision may be exercised over Home Care Nurses, Visiting Nurses, Home Health Aides and other support staff.

QUALIFICATIONS

Master's degree in adult or geriatric nursing is necessary.
Graduation from a Nurse Practitioner Continuing Education Program is necessary.
Certification as a Nurse Practitioner in adult or geriatric nursing by a National Credentialing Organization is necessary.
Demonstrated ability to work autonomously in a non-structure environment is necessary.
Ability to meeting bonding criteria is necessary. Current valid, unrestricted driver's license is necessary. Proof of current CPR certification and negative TB test is necessary.
Two years experience as an RN including home health experience is desirable.
This position requires Primary Source Verification.
Current license as a Registered Nurse in the State of Michigan is necessary.
