BASIC FUNCTION AND RESPONSIBILITY Provides clinical expertise, in collaboration with a multidisciplinary team, to coordinate and manage care delivery to ensure an efficient and cost-effective course of hospitalization and an appropriate, timely discharge plan. Utilizes nursing process, specialty expertise and management principles to ensure timely, appropriate and cost-effective movement of the patient through the health care continuum with emphasis on the inpatient stay. Responsible for identifying and completing arrangements for a variety of related services for patients and families of the University of Michigan Hospitals and Health Centers. CHARACTERISTIC DUTIES AND RESPONSIBILITIES
Utilize the
nursing process to assess, identify, plan, negotiate and
implement continuing patient care including home care, infusion,
extended care facility, hospice and a variety of other community
resources. SUPERVISION RECEIVED Direct supervision is received from the Director of Practice Management. Designated service chiefs provide functional supervision.

Bachelor’s
degree in Nursing (BSN) is required. |
