TITLE: PRACTICE MANAGEMENT COORDINATOR GRADE: N3
 

BASIC FUNCTION AND RESPONSIBILITY

Provides clinical expertise, in collaboration with a multidisciplinary team, to coordinate and manage care delivery to ensure an efficient and cost-effective course of hospitalization and an appropriate, timely discharge plan. Utilizes nursing process, specialty expertise and management principles to ensure timely, appropriate and cost-effective movement of the patient through the health care continuum with emphasis on the inpatient stay. Responsible for identifying and completing arrangements for a variety of related services for patients and families of the University of Michigan Hospitals and Health Centers.

CHARACTERISTIC DUTIES AND RESPONSIBILITIES

Utilize the nursing process to assess, identify, plan, negotiate and implement continuing patient care including home care, infusion, extended care facility, hospice and a variety of other community resources.
Provide education, consultation, and information to the health care team and/or patient/family on continuing care options.
Orchestrate care between multiple care delivery sites and between multiple caregivers.
Collaborate with the multidisciplinary team in the development and implementation of population based continuing care protocols for continuity of care purposes.
Collaborate with the multidisciplinary team to ensure appropriate levels of patient care and cost effective use of hospital resources.
Communicate various aspects of the discharge planning process to patients and families, other care providers, clinical specialists, support staff and Utilization review coordinators.
Collaborates and shares information with the Utilization Review Coordinators by providing clinical advice and consultation regarding patient’s medical condition, level of care and evaluation of clinical outcomes.
Leads the care team in formulation of care options by incorporating pertinent reimbursement, resource utilization, and benchmarking data.
Utilizes the Continuous Quality Improvement Process (Plan-Do-Check-Act) to improve population based operations and practice as well as own delivery of services.
Serves as a liaison between continuing care or community providers and UMHHC.

SUPERVISION RECEIVED

Direct supervision is received from the Director of Practice Management.  Designated service chiefs provide functional supervision.

No supervision exercised.

QUALIFICATIONS

Bachelor’s degree in Nursing (BSN) is required.
Current licensure as a Registered Nurse in the State of Michigan is required.
Five years of nursing experience, three years of which include nursing management or case management experience and two years experience as a discharge planner or in a home care setting is required.
This position requires Primary Source Verification.
