TITLE:  CLINICAL CARE COORDINATOR GRADE: N3
 

BASIC FUNCTION AND RESPONSIBILITY

To provide expert nursing care in multiple units to a specified patient population; to design and coordinate nursing care programs in multiple units to a specified patient population; and to provide instruction and consultation to members of the health care team.

CHARACTERISTIC DUTIES AND RESPONSIBILITIES

Provide expert nursing care to a specific patient population through individual assessment, evaluation and monitoring of treatment in multiple units.
Coordinate and assess the nursing care given to a specific patient population in multiple hospital or clinic units.
Design, monitor and evaluate nursing care programs and individualized patient care plans.
Provide liaison and consultation in the area of clinical expertise to members of the health care team in multiple units, services and disciplines.
Participate in daily rounds with medical and nursing staff.
Document patient care and progress and maintain detailed patient and practice records.
Design, develop and maintain patient education programs.
Initiate and participate in designing and conducting conferences and in-service programs for staff and students.
Participate in the development and maintenance of clinical area quality assurance programs and activities.
Participate in research studies by identifying research subjects and evaluating data.

SUPERVISION RECEIVED

General supervision is received from a Director or Assistant Director of Nursing, Head Nurse or other designated official.

SUPERVISION EXERCISED

Functional supervision may be exercised over Clinical Nurse I's, II's, Allied Health, Service/Maintenance and other support staff.

QUALIFICATIONS

Bachelor's degree in Nursing is necessary.
Current licensure as a Registered Nurse in the State of Michigan is necessary.
Three or more years of nursing experience as an R.N. is necessary.
Knowledge of the clinical specialty is desirable.
This position requires Primary Source Verification.
