Our goal was to establish a protocol to return patients to their local nephrologist six months post kidney transplant. This will increase referring physician satisfaction and encourage future referrals. We created a committee who developed a detailed flow chart to show the process for referring a patient back. This includes notifying local nephrologists at various checkpoints about the process as well as informing them of any extenuating circumstances that may keep a patient at the University of Michigan Transplant Center beyond the six month benchmark.
