Provide skilled home care nursing services during intermittent visits to multiple patients on a daily basis; assess physical, psychosocial and environmental needs of patients and families in accordance with agency, state and federal regulations; coordinate care for assigned caseload of patients and families as a member of a multi-disciplinary home health team. Perform comprehensive initial and ongoing nursing assessment to identify the physical, psychosocial, behavioral and environmental needs of the patient.
