Currently there are 3 Transition of Care Pharmacy Technicians in this role. We hope to expand to 12 Technicians in order to expand our services to the children's hospital and to take on more responsibilities such as medication reconciliation. The pilot has provided very promising data, confirming the effectiveness of the TOC role in reducing the number of patients who experience access barriers to their discharge medications and has also proven that TOC reduces the amount of time that patients wait at our pharmacy for discharge medications.
