New Employee Health Assessment

Occupational Health Services (OHS) welcomes you to Michigan Medicine at University of Michigan! As part of your onboarding, your immunization records will be reviewed by OHS as required by Michigan Medicine Policy 04-06-002. Please carefully read and follow all instructions provided below. The New Employee Health Assessment must be completed prior to any patient contact, or within one (1) month from date of hire, whichever is sooner.

Action Items:

  1. A Michigan Medicine Medical Record Number (MRN) is required. If you do not already have one, call Michigan Medicine Patient Registration at 734-936-4990 or toll free 866-452-9896 to obtain your medical record number.

  1. Complete this NEHA Immunization Form and save a copy.

  1. Obtain a copy of your previous immunization record. (See the NEHA Form above for the list of specific immunization requirements).

  1. Send one email with the completed NEHA Form and your previous immunization records attached to MM-OHS-NEHA@med.umich.edu

  1. You must submit your Covid-19 immunization records through Wolverine Access prior to your start date.

All new employees are required to have a baseline tuberculosis test. OHS staff will provide further instructions regarding where to obtain a TB test. Job requirements may require a N95 fit test or other testing specific to your job duties. Please consult with your supervisor to identify any additional requirements.

Please allow up to ten business days for OHS to process your immunization records. You will receive a follow-up email outlining any next steps. Michigan Medicine employees can access their Health Rx portal to check immunization compliance status.
