If you were eligible for health benefits in 2016, the university will provide you with a required tax document in early 2017. Form 1095 reports information about your health care coverage as required by the Affordable Care Act. Please visit Wolverine Access by January 8, 2017 to consent to receive an electronic PDF copy of your 2016 Form 1095 instead of an identical mailed copy.
Go to Wolverine Access > Faculty & Staff tab > Employee Self-Service > Benefits > ACA 1095 Consent to provide your consent. Alternatively, if you will receive a W-2 from the university for 2016, you can consent to receive both forms electronically at the same time (until January 8) in Wolverine Access > Faculty & Staff tab > Employee Self-Service > Payroll and Compensation > View Paycheck.
Form 1095 will be available electronically starting January 23, 2017 in Wolverine Access > Employee Self-Service > Benefits > View Form 1095. If you prefer to receive a hard copy or do not provide your consent to receive an electronic copy by January 8, the university will mail your 1095 to your current address on file by January 31, 2017.
