Procedure: Expanded Long Term Disability Enrollment and Application

Eligibility: Type of Appointment
Your appointment must last or be expected to last at least eight or more continuous months.

WHEN YOU MAY ENROLL

50% or Greater Appointment, Less than Four Years of Service

50% or Greater Appointment, Four or More Years of Service Less Than 50% Appointment, Five or More Years of Service

You may enroll as soon as you meet the eligibility requirements. Enrollment during the first four years is optional, at your expense, and based on your entire salary.

The University pays for the first $30,000 of your base salary.

Coverage over $30,000 is optional and at your expense.

* Supplemental Faculty, Graduate Students, House Officers, Professional Specialists, AFSCME, and Research Fellows are not eligible for this plan.


Procedure A: Enrollment procedures for the Expanded Long-Term Disability Plan for new hire and/or newly eligible employee with Less than Four Years of Service

RESPONSIBILITY ACTION
Benefits Office

The Expanded Long-Term Disability enrollment materials are mailed to new hire and/or newly eligible employees at his/her home address. This package contains the employee's personalized enrollment worksheet (PEW).

Employee

Upon receipt of the personalized enrollment worksheet (PEW), the new hire or newly eligible employee may begin the process of enrolling in benefits plans using their Personal Enrollment Worksheet, as described in the Enrollment section of the Benefits website.

The new hire and/or newly eligible employee have 60 days from new hire date and or newly eligible date to enroll in their benefits.

Return all pages of your completed Personal Enrollment Worksheet using the instructions on the form or on the website.

If you wait past 60 days of the date of hire or newly eligibility date to enroll in the Expanded Long-Term Disability benefit you must also submit a completed Statement of Health form and application and be approved for enrollment.

Expanded Long-Term Disability Applications with Health Statements are available on the Benefits website.

The LTD external Administrator must approve your application and Statement of Health before coverage takes effect.

Coverage takes effect:

  • The date your Statement of Health is approved by the external Administrator

Benefits Office

Review application for correctness and send to the Benefits Transaction Team for processing.

  • Coverage will be effective the date of hire or the first day of the newly eligible appointment or,
  • If your application is not received within 60 days, coverage begins the date your enrollment with a Statement of Health is approved.
Employee

In all cases, employee must be actively at work before coverage can take effect.

Benefits Transaction Team (BTT)

BTT inputs enrollment and processes the event.

Benefits Office

Mail confirmation statement to the employee home address on record.

Employee

Review pay stub to validate that enrollment is complete.

Review personalized Confirmation Statement to confirm enrollment. Notify Benefits Office if enrollment is not indicated on pay stub.

Procedure B: Enrollment procedures for the optional coverage in the Expanded Long-Term Disability Plan for an Eligible Employee with Four or more years of service Or an Eligible Employee with Five or More years of continuous service

RESPONSIBILITY ACTION
Benefits Office

Automatically enrolls eligible employees who have four or more years of continuous service in the University paid Expanded Long Term Disability Plan coverage. The University-paid coverage provides LTD coverage on the first $30,000 of annual base salary. Coverage continues as long as the employee maintains eligibility.

Employee must follow enrollment procedures on How to Enroll for coverage on annual base salary above $30,000 if they did not enroll in optional employee-paid coverage during their first four years of eligible appointment.

The Benefits Office to request an application to enroll in the Expanded Long-Term Disability with a Health Statement or access the form on the Benefits Office website.

Validate eligibility to enroll with a health statement.

Mail application with health statement to employee.

Employee

Complete the LTD application and the health statement and return to the Benefits Office.

Benefits Office

Review application for correctness. Mail health statement to the LTD external Administrator for consideration for approval.

Coverage takes effect:

  • the date your Statement of Health is approved by the external Administrator
Employee

Must be actively at work before coverage can take effect.

If not approved, notify employee in writing of denial and/or need for additional information.

Procedure C: Procedures to Apply for Expanded Long-Term Disability Benefits :

Work~Connections (W~C)

The W~C team will work with you to obtain the necessary medical documentation. They will coordinate your application for Long-Term Disability with the Benefits Office. When appropriate, W~C will refer to the Benefits Office to begin the LTD application process.

Benefits Office

Will contact employee to and mail LTD application forms once appropriate referral is received from W~C.

Employee

Return LTD application forms to the Benefits Office. Retain copies of forms.

Benefits Office

Review LTD application forms and medical documentation. Initiate action to obtain additional documentation as needed. If LTD file is complete, forward claim to the LTD external Plan Administrator for review and determination (approval or denial of LTD benefits).

Communicate determination to employee. UHR, HRRIS, Department, and W~C.
