Procedure A: Reinstatement |

| Faculty or Staff Member | Apply in writing after three continuous years of service. |
| Academic and Staff HR Services, Health System HR or Regional HR office | Determine and approve eligibility. Notify HR Records and Information Services of Approval. Notify faculty or staff member of approval in writing. Include reinstated service date and the effective date of the change. Send copies to Payroll/Timekeeping, Benefits Office, Employee’s department, and Service Awards (staff only). |
| HR Records and Information Services | Adjust service date and benefit eligibility date. |
| Human Resources, Service Awards | Arrange for staff member to receive the appropriate award based on the adjusted service date. |
Procedure B: Credit for Prior Service |

| Faculty or Staff Member | Apply in writing after ten years of service. |
| Academic and Staff HR Services, Health System HR or Regional HR office | Determine and approve eligibility. |
| HR Records and Information Services | Create a person modifier. |
| Human Resources, Service Awards | Arrange for staff member to receive the appropriate service award. |
