Procedure: Leaves of Absence

APPLIES TO: Non-probationary, regular Allied Health, Office, Professional/Administrative, and Technical employees

RESPONSIBILITY ACTION
Employee

Submit a signed Request for Leave of Absence Form (form #36609) accompanied by appropriate documentation to supervisor.

Supervisor

Review the request to determine the employee's eligibility for the leave of absence. If appropriate, determine with Payroll Timekeeping the employee's sick and vacation balances to establish the start date of the leave of absence.

If the request does not meet the eligibility requirements of this policy, send the Request for Leave of Absence Form, and any supporting documentation, to the appropriate Human Resources office for review.

If the request has supervisory approval, complete page 2 of the Request for Leave of Absence Form. Send the Request for Leave of Absence Form, and any supporting documentation, to the appropriate Human Resources office for approval.

If the employee's pay for the current pay period will be affected, inform the Payroll Office of the impending change of status.

Human Resources Review the Request for Leave of Absence Form, and supporting documentation, for conformity to regulations and policy. Contact the responsible supervisor if questions exist.

If the leave of absence is approved:

a) Notify the employee of the approval in writing.

b) Forward the Request for Leave of Absence Form and supporting documentation to HR Records and Information Services or the Health System Human Resources Office.

If the leave of absence is not approved, return the request with an explanation to the employee's department.

HR Records and Information Services or Health System Human Resources Office

Process the Request for Leave of Absence.
