*To earn the cash incentive, you must complete all three steps of the wellness assessment:

  1. Sign up and schedule your screening appointment
  2. Complete the health questionnaire
  3. Attend a 30-minute wellness screening

StayWell will provide the University of Michigan with a list of participant names who have completed all three steps of the MHealthy Wellness Assessment. If you are eligible for the cash incentive you will receive it in your paycheck about 60 days after the date of your wellness screening.
