MHealthy Wellness Assessments Accepting Walk-Ins Beginning April 14

Beginning April 14, MHealthy Wellness Assessments will accept walk-ins at all screening sites.  Please go to www.hr.umich.edu/mhealthy/programs/wellness/pdf/walkins.pdf for a complete list of sites.

MHealthy still encourages scheduling an appointment in advance, as people with appointments will be taken first so they can complete the screening within 30 minutes. To schedule a wellness screening appointment, go to www.mhealthy.umich.edu. The last screening date is April 24, 2009.

Walk-in appointments are available for those who may have missed their appointment, could not find an appointment that fit their schedule or are unable to schedule a specific date and time because of changes in their calendar.  The length of the appointment will depend on the number of unscheduled participants arriving at the screening site.

To expedite the screening process, please complete the StayWell online Health Questionnaire, available at https://umich.online.staywell.com/includes/login/index.aspx, prior to the screening appointment and bring the StayWell User ID and Password you created. For questions about the online Health Questionnaire, call StayWell’s HelpLine at (800) 926-5455.

More than 17,000 eligible faculty and staff have already received or are registered for their free wellness assessment.  Participants learn their cholesterol, blood pressure and glucose levels and talk with a health professional to identify health risk factors.  Benefits-eligible faculty and staff members hired before March 1, 2009, who complete their wellness assessment, qualify for a $100 pre-tax cash incentive.

For more information on MHealthy Wellness Assessments, eligibility requirements or incentive payment, please visit www.MHealthy.umich.edu.

Is Your Beneficiary Designation Up to Date?

It is important to keep your beneficiary designations for your U-M Life Insurance and your U-M Retirement Savings Plan accounts with TIAA-CREF and Fidelity Investments up to date.  Family status changes, such as marriage, divorce, birth, or adoption, may affect your desired beneficiary intentions.  It is recommended that you review and update your beneficiaries periodically to make sure that they reflect your wishes as your circumstance change.  Information on how to update your beneficiary designations may be found on the Benefits Office Web site at: www.benefits.umich.edu/events/beneficiary.htm

Please note that updating your beneficiary for life insurance does not update it for your retirement savings plan accounts. You must complete a separate beneficiary designation form for each plan in which you are enrolled.

New I-9 Form for Employment Eligibility Verification Available at HRRIS Web Site

U.S. Citizenship and Immigration Services (USCIS) recently required a revised I-9 form be used for employment eligibility verification. The new form is required for all new hires as of April 3, 2009 and is available on the HRRIS site, here: http://www.hr.umich.edu/hrris/forms/pdfs/I-9.pdf. The main difference in the revised Form I-9 is that all documents presented during the verification process must be unexpired.

NOTE: If you maintain a Web site with hiring information for your unit, please be sure to update your I-9 link to the above URL. In this way, you can be sure to have access to the appropriate form. Please do not save the form to your server. This can lead to the possibility of outdated forms being used.

There are two quick ways to recognize if you have the correct form:

  1. In the lower right hand corner is the revision date of 2/2/09 (“Form I-9 Rev. 02/02/09”).
  2. Also, there are four (4) boxes as options in Section #1 instead of the three (3) boxes on the old form.

If you click on http://www.hr.umich.edu/hrris/forms/pdfs/I-9.pdf and still see the old I-9 form, please click the “refresh” button on your browser.

Questions about the I-9 Form?
Contact, David Day in University Human Resources at (734) 763-2387.

Work/Life Resource Center Announces “Family Helpers Program”

Ever wonder how you could connect with a babysitter, dog walker or house sitter?  Need some help with raking leaves or cleaning out the gutters?  The Work/Life Resource Center is pleased to announce the new Family Helpers program that connects U-M students interested in occasional employment with staff and faculty who are in need of a variety of assistance

Family Helpers lists students interested in providing:

  • Occasional Child Care
  • Occasional Care For Elderly Or Disabled Relatives
  • Pet Sitting
  • House Sitting
  • Gardening/Yardwork
  • Errands
  • Tutoring
  • Housekeeping/Cleaning

Student profiles include information on their past experience, special certifications (such as CPR/First Aid) and training relevant to the jobs they seek. Work/Life Resource Center has confirmed that all students are currently enrolled in U-M and have cleared a State of Michigan criminal background check.

You can access the Family Helpers listing at https://www.hr.umich.edu/familyhelpers.

Currently, there are approximately 40 students who have listed their information on the Family Helpers website, from the School of Nursing, School of Education, School of Art and Design and the Department of Psychology.  These listings will be updated twice each year.

*Please note that all negotiations related to occasional employment, including setting an hourly rate, are handled directly between the staff/faculty member and the student. The Work/Life Resource Center makes this listing of students available to you but does not facilitate, negotiate or take responsibility for the working relationship between you and the student.
